It looks like tetra-js.php (the Tetra JavaScript file) isn't getting loaded by your browser. Check the head section of your HTML page for a script tag similar to this:

<script src="tetra-js.php"></script>

Select : I can't find the script tag
Select : Okay, I see the script tag
